Overview

Modafinil is a wakefulness-promoting agent (eugeroic) approved for narcolepsy, obstructive sleep apnea, and shift work sleep disorder. It is widely prescribed off-label for fatigue, ADHD, and cognitive enhancement.

Mechanism of Action

Mechanism is not fully understood. Modafinil weakly inhibits the dopamine transporter (DAT), increasing extracellular dopamine. It also affects orexin/hypocretin, histamine, norepinephrine, serotonin, and GABA systems involved in wakefulness regulation.

Taper Notes

Low classical dependence liability, but rebound fatigue and unmasking of underlying sleep pathology are common on discontinuation. A 2–4 week stepdown reduces rebound severity and allows the underlying condition (OSA, narcolepsy, shift-work disorder) to be addressed independently.

  • 1Schedule the taper during a period of lower functional demand for the patient — rebound fatigue is clinically significant even when classical withdrawal is not.
  • 2If modafinil is treating an underlying sleep disorder (OSA, narcolepsy), confirm that condition is independently optimized (CPAP adherence, scheduled naps) before discontinuation.
  • 3Behavioral measures — hydration, morning light exposure, fixed wake time — substantially blunt the rebound period and should be in place before reductions begin.
  • 4Modafinil is a CYP3A4 inducer; counsel patients on hormonal contraception to maintain backup contraception during therapy and for one month post-discontinuation while enzyme activity normalizes.

Interactions & Safety

Drug Interactions

  • Hormonal contraceptives — modafinil induces CYP3A4 and reduces contraceptive efficacy; use additional contraception during and for 1 month after
  • Cyclosporine and other CYP3A4 substrates — reduced exposure
  • Warfarin — monitor INR (modafinil affects CYP2C9)

Food Interactions

  • Food delays Tmax by ~1 hour but does not affect total exposure
  • Avoid combining with high doses of caffeine or other stimulants

Contraindications

  • Known hypersensitivity to modafinil or armodafinil
  • History of left ventricular hypertrophy or mitral valve prolapse with prior stimulant-related symptoms

Toxicity

Severe rash including Stevens-Johnson syndrome (rare). Cardiovascular effects (tachycardia, hypertension). Psychiatric effects including mania, psychosis, and suicidal ideation.
